Our anxiety quiz is based on the Generalized Anxiety Disorder 7-item scale (GAD-7), developed by Spitzer, Kroenke, Williams, and Lowe in 2006.
What It Measures
The GAD-7 assesses seven core symptoms of anxiety over the past two weeks:
- Feeling nervous, anxious, or on edge
- Not being able to stop or control worrying
- Worrying too much about different things
- Trouble relaxing
- Being so restless it's hard to sit still
- Becoming easily annoyed or irritable
- Feeling afraid as if something awful might happen
Severity Levels
GAD-7 scores are interpreted as follows:
- 0-4: Minimal anxiety
- 5-9: Mild anxiety
- 10-14: Moderate anxiety
- 15-21: Severe anxiety
Validation & Reliability
The GAD-7 is one of the most rigorously validated anxiety measures:
- Sensitivity: 89% at cutoff score of 10
- Specificity: 82% at cutoff score of 10
- Internal Consistency: Cronbach's alpha of .92
- Test-Retest Reliability: Intraclass correlation of .83
- Validation Sample: Over 2,700 primary care patients

Limitations
While the GAD-7 is highly validated, it has important limitations:
- Screening Focus: Primarily screens for Generalized Anxiety Disorder, not all anxiety types
- Self-Report Bias: Results depend on honest self-assessment
- Symptom Overlap: Anxiety symptoms can overlap with depression, medical conditions, and substance use
- Point-in-Time: Measures symptoms over past 2 weeks, not lifetime patterns
